Who is a suitable candidate for tummy tuck (abdominoplasty) surgery?

The key criterion sought in patients intending to undergo tummy tuck surgery is presence of excess skin in the abdomen. People with excess skin may also have stretch marks on the skin.

Formation of stretch marks is at the same time indicative of deteriorated skin quality, along with excess skin. Another surgical indication for tummy tuck is that the patient’s belly bulges significantly due to diastasis recti.

Although these changes often occur after pregnancy, they can also be seen in people who have not had a pregnancy but have gained and lost a lot of weight rapidly, which is the reason it may sometimes be necessary to perform tummy tuck surgery on younger people who have not given birth.

What steps need to be taken before tummy tuck surgery?

As in every surgery, preop tests must be run and the patients must be examined by an anesthesiologist before tummy tuck surgery. Usually, these procedures are done the day before the surgery.

Also, patients are required to sign the necessary surgery consent forms the day before surgery. Preop photographs are taken of each patient. On the morning of surgery, necessary drawings and planning is made for the upcoming tummy tuck and for other surgeries, if any, to be performed in combination.

For a smooth and good wound healing after tummy tuck surgery, patients must avoid smoking for 4 weeks before and 4 weeks after the surgery. Intake of aspirin is disadvised one week before surgery as it can increase bleeding.

Also, contraceptives should be discontinued 4 weeks before tummy tuck surgery, as they create a pregnancy-like effect and have negative effects on bleeding and coagulation systems. Since green tea consumption increases bleeding, it is recommended to stop consuming green tea a few weeks before surgery.

How long does a tummy tuck surgery take?

Tummy tucks are usually performed in combination with liposuction. I prefer to combine all tummy tuck surgeries with liposuction.

I try to achieve a nice trim in the waist area, by performing liposuction especially on the side parts of the abdomen, the back area and the lower parts of the waist. A tummy tuck surgery combined with liposuction takes around 4 hours on average.

How is tummy tuck surgery done?

I start tummy tuck surgeries with liposuction on the back and lower parts of the waist, during which the patient lies on the operating table in prone position so that the fat in these areas can be removed. The patient is then turned to supine position.

While the patient is lying on their back, liposuction is applied to the sides of the breasts in a way that extends to the sides of the abdomen and the armpit area so as to create a thoroughly trimmed waistline.

If the patient is a non-smoker, liposuction may also be applied to trim the anterior abdominal area. In smokers, however, liposuction on the anterior abdominal area is associated with high risks as it will cause wound healing problems.

Then, the skin around the navel, which is connected to the abdominal muscles at its base, is cut to release it from the abdominal skin. The planned amount of skin and adipose tissue is then cut out and the skin above the navel is released by opening a tunnel up to the thorax.

The separated abdominal muscles are brought closer to each other with sutures to tighten the abdomen. At this point, I inject a local anesthetic into the abdominal muscles to relieve the muscle pain that patients may have postop.

Then, the skin above the navel is pulled down and stitched to the lower incision line such that the abdominal incision scar will be hidden under the patient’s underwear.

Before the incision line is sewn closed completely, an elliptical hole is cut in the abdominal skin to accommodate the navel in its new position, through which the navel will pass through before it is stitched to the surrounding skin.

Lastly, drains are inserted before finishing the surgery, with 2 in the back and 2 in the abdomen. Special plasters are applied dress the incision lines and shapewear is put on the patient. The patient is transferred to their room after full recovery from anesthesia.

What are the points to consider after tummy tuck surgery?

Patients can have their first meal 4-5 hours after the end of surgery. This meal is usually liquid food in the form of soup, whereafter the patient can slowly start to sit up in bed so that their blood pressure can recover.

Once the effects of anesthesia have completely faded, patients start short walks, at which point they should walk with their bodies bent slightly forward from their waist upwards.

The day after the surgery, the patient’s initial dressings are replaced by smaller dressings whereafter their shapewear is put back on. I recommend our patients to stay in the hospital for 2 days after tummy tuck surgery.

The drains are removed 6 to 7 days after surgery and patients begin to move more easily. Once the drains are removed, patients can start walking upright and take a bath without problem 2 days after. The sterile dressings are removed to check on the wounds 3 weeks after surgery.

If there is no healing problem, patients are instructed to start applying silicone gel twice a day to fade the scars. I recommend using the silicone gel for at least 3 months. Mild exercises like cardio type exercises can be started 4 weeks after surgery.

I advise that patients who do routine sports wait 6 weeks before they resume their usual daily sports activities. Especially in patients who take care of their diet after surgery and do regular sports, the trimming achieved in the waist area can be really spectacular.

Therefore, as part of preop interviews, I try to motivate my patients and explain them in detail how and what lifestyle changes they need to make after surgery.

What is a mini tummy tuck?

A mini tummy tuck is a surgery where we remove the excess skin under the belly button. Also, stitches can be applied to tighten the muscle areas under the belly button.

The procedures done in a mini tummy tuck involve the area under the belly button, meaning that there will be no incision around the belly button and that its position will not change much but only shift slightly downwards as the skin to be removed is only the skin positioned under the belly button.

What is an endoscopic tummy tuck (endoscopic abdominoplasty)?

Endoscopic tummy tuck is a type of surgery with minimal scarring where we operate on the abdomen without any incision and without removing any excess skin.

The procedure involves tightening and reshaping of endoscopically elevated abdominal muscles with the help of sutures. It is a method that can be applied for patients who do not have sagging or excess abdominal skin, but only a separation or looseness in abdominal muscles.

Can I get pregnant after tummy tuck surgery?

Abdominoplasty does not preclude pregnancy. It does not prevent the baby’s growth in the mother’s womb and patients can carry their child to term healty.

Unless the patient gains weight very rapidly during pregnancy giving rise to stretch marks on the abdomen, the abdominal area will return to its pre-pregnancy shape after pregnancy.

But if the patient has gained a lot of weight rapidly and stretch marks have developed on the abdominal skin during pregnancy, then the tummy tuck should be revised at the earliest 1 year after delivery.

What risks does tummy tuck surgery involve?

The most common complication after abdominoplasty is problems in wound healing. This risk is higher if the patient is overweight, if tummy tuck was performed very aggressively, if the patient has an additional disease or if the patient is a smoker.

Wound healing problems are usually seen in the area beneath the navel, which is generally treated with local wound care.

Is tummy tuck surgery painful?

Pain after tummy tuck surgery can vary from person to person. Generally, patients try to compare this pain with the pain after a c-section. In tummy tuck surgery, the muscles are not cut but are brought closer to each other with the help of stitches, which means that there will be less pain after surgery than after a c-section.

But this does not mean that a tummy tuck is completely painless. To reduce this pain, I inject drugs with local anesthetic effect into the abdominal muscles during surgery.

The strong painkillers we administer to our patients at the end of surgery provide a significant pain relief. Patients begin to feel less pain as they start walking in the forthcoming days.

Will there be any scars after tummy tuck surgery?

There will be scars after tummy tuck surgery, but they will be hidden under the patient’s panties. Patients are instructed to start using silicone gel 3 weeks after surgery to fade the scar and make it less prominent. I recommend using the silicone gel twice a day for at least 3 months.

The scar will fade over time and become less visible 1 year after surgery. One year after the surgery, patients can get their scar tattooed with skin-colored ink to make their scar even less visible, which we refer to as scar camouflage.

Can tummy tuck and liposuction be done together?

Tummy tuck surgeries are mostly performed in combination with liposuction. Liposuction is applied to the abdomen, sides, waist and back areas to trim the waist circumference more beautifully and give the patient an hourglass-shaped waistline.

I combine tummy tuck surgery with liposuction in almost all of my patients. However, the patients’ blood counts ​​must be high enough in order for these surgeries to be performed together.

For this reason, I prefer to see my patients’ blood counts first before I decide on the way to go. Tummy tuck surgery and liposuction can be easily combined for patients with normal or slightly above normal blood values.

Can tummy tuck remove all postpartum stretch marks?

The amount of stretch marks that can be removed with tummy tuck varies according to the amount of excess skin. To give a general idea, all patients get rid of stretch marks under the navel.

In patients with lots of excess abdominal skin, it is possible to get rid also of stretch marks above the navel to a certain extent, while some of the stretch marks above the navel that can’t be removed will remain under the patient’s underwear and thus become less visible after surgery.

Can umbilical hernia be treated with tummy tuck surgery?

Umbilical hernia, if any, can be treated during tummy tuck surgery, which means that a patient’s umbilical hernia can be treated and his/her abdominal muscles can be tightened in a single session.

When combined, these two procedures can make the abdominal wall muscles even stronger, giving the patient a much tighter abdomen after surgery.

Can tummy tuck surgery be done in summer?

Tummy tuck can be performed in any season of the year. The important thing here is that the patient will have to wear shapewear for 6 weeks after surgery.

So, tummy tuck surgery can be performed also in summer if it is possible for the patient to spend their recovery time in a cool place after surgery. For patients who want to take a vacation during the summer months, I recommend tummy tuck surgery after their vacation.
