Nose is the organ that is located in the centre of the face and takes the attention when another person looks at the face. Nose job (rhinoplasty) not only corrects abnormal shape of the nose, but it also helps certain problems that prevent breathing easily, such as deviation.
Rhinoplasty turkey may involve procedures that are performed simultaneously for two objectives. First, the deviated parts of the nose are corrected and obstructions caused by bones, cartilages or nasal turbinate are eliminated in order to ensure breathing easily and restore physiological function of the nose.
The second objective is to create a nose with shape and size that fits and also proportional to face of the patient. I perform all nasal surgeries with due consideration of both objectives.
How is nose job performed?
A good plan is required to obtain positive outcomes in nose job. For this purpose, I prefer to inform my patients by showing changes I want to make through every kind of technological opportunity and target postoperative outcome in the preoperative period. Although I generally use open surgery technique in rhinoplasty, I also commonly apply closed rhinoplasty technique for suitable patients.
Recently, an ultrasonic device (piezotome) that is the most valuable and positive development for rhinoplasty procedures is, now, an inevitable part of my surgical technique. Now, I use ultrasonic rhinoplasty technique to carry out bone shaping which had been done with surgical hammer, chisel and rasper previously.
The most important quality of ultrasonic rhinoplasty are that the device only focuses on bone tissue, which means the nasal cartilages, mucosa and vessels are not damaged, both bruising and edema are pretty minimal and a short and rapid recovery is achieved.
Overnight hospital stay is highly important for patients to provide a good postoperative care and facilitate patient safety. I prefer to remove splint and nasal pack we place during the surgery to maintain the shape of the nose on Postoperative Day 7, although it is subject to interpatient variations.
We aim to duly inform our patients about rhinoplasty which takes about 2-2.5 hours and postoperative care through consent and information forms we provide in the preoperative period.
Who are good candidates of rhinoplasty?
Rhinoplasty usually involves correction of abnormalities in bone tissue. Therefore, it should be performed after maturation of bones. This achieved around age of 10 years in most patients. If patients do not have a serious severe deformity or a breathing problem, rhinoplasty is scheduled after the patient is 18 years old.
Since bone maturation is a bit quicker in women, rhinoplasty can be performed in female patients around 17 upon consent of parents. However, I believe that it is necessary to wait for psychological maturation of patients. Therefore, I recommend to perform rhinoplasty after 18.
Things you need to know about rhinoplasty:
Before rhinoplasty is performed, routine preoperative tests are done and assessment is made by anesthesiologist for each patient. I prefer to make one more interview with my patients one day before the surgery, have them sign consent forms and take photos preoperatively.
I recommend patients, who smoke, to quit smoking at least 4 weeks before rhinoplasty and for 4 weeks after rhinoplasty. In this way, patients wake up well and they are easily recovered from general anesthesia. Also, postoperative swelling is milder.
What are considerations after rhinoplasty?
To reduce effects of general anesthesia, patients stay in hospital the night of the rhinoplasty surgery. They are discharged one day after the surgery and they are prescribed painkiller, antibiotic and nasal spray . I prefer to remove plaster cast and nasal packing 1 week after the surgery.
I attach special tapes to the nose after the plaster cast is removed. These tapes must remain in place for 1 week. Patients should protect their noses from traumas for 6-7 weeks after the surgery. In this period of time, bone healing is completed and the nose is further stabilized.
I recommend patients to avoid wearing eyeglasses for 6-7 weeks. Generally, 80% of the swelling disappears by 2 months after the surgery. Generally, the nose takes its final shape about 1 year after the surgery. At the end of one year, the shape of the nose is stable.
What are types of rhinoplasty?
In terms of technique, there are two types of rhinoplasty. The first one is open technique, while the other is closed rhinoplasty.
Recently, ultrasonic rhinoplasty gained popularity especially after technology is more widely used in the field of plastic surgery. In this method, nasal bones are shaped with an ultrasonic device, called piezo. Ultrasonic rhinoplasty can be performed with both open and closed technique.
Ultrasonic rhinoplasty uses an ultrasonic device, called piezo, to shape nasal bones and remove osseous part of the nasal hump.
Most obvious advantage of the device is that it affects only the bone tissue. Thus, bone structures are more clearly shaped. Thus, nasal blood vessels, nerves and other structures are not harmed. Therefore, swelling and bruising will be milder after the surgery. Postoperative recovery is faster. Thus, patients can resume routine activities of daily life quicker.
Is hospital stay required after rhinoplasty?
Rhinoplasty is a surgery that must be performed under general anesthesia. This issue is highly important for patient comfort. As is the case with all surgeries performed under general anesthesia, patients are recommended to stay in hospital for one night after rhinoplasty. Here, the purpose is to completely recover patients from effects of general anesthesia.
Is open or closed surgical method best in rhinoplasty?
This issue may vary from patient to patient. It is more advantageous to apply closed surgical technique in patients for whom better outcomes can be obtained with closed technique. It is better to use open surgical technique for patients with severe nasal curvature or deformity. In secondary or tertiary nasal surgeries, open surgical technique is preferred most.
Is there any other method to alleviate swelling and bruising after rhinoplasty?
To alleviate swelling and bruising, cold pack is applied to the eye for 10 minutes at one hour intervals for 48 hours after the surgery.
Should nasal splint be used after rhinoplasty?
Nasal splint should be used if nasal bones have been shaped on rhinoplasty. Here, the purpose is to achieve smooth nasal bone healing. Also, nasal splint provides protection for the nose in early postoperative period. Nasal splint is placed onto nasal bones. It is removed 7-10 days after the surgery. There is no need for nasal splint if nasal bones have not been intervened on rhinoplasty.
Does pain occur after rhinoplasty?
Recently, I had no patient who complained of pain after rhinoplasty. Almost none of my patients complained of pain especially after I started using ultrasonic rhinoplasty technique. Therefore, contrary to other surgeries, I can emphasize that rhinoplasty is a pain-free surgery.
Is silicone nasal packing always inserted into nasal cavity after rhinoplasty?
Silicone packing is necessary for patients, whose nasal deviation is corrected surgically. The nasal packing aims to keep the mid-line cartilage (septum) of the nose where it should be and to prevent bleeding. Nasal packing is removed 7 days after rhinoplasty.
May rhinoplasty be performed in people who have history of nasal filler procedure?
Rhinoplasty can be performed in patients who have history of nasal filler. Fillers of the patient are removed in the surgery. Here, it is important for the patient to inform the doctor during preoperative evaluation. Thus, preoperative evaluation can be properly done.
Does removal of nasal packing cause pain after rhinoplasty?
Since nasal packing is silicone, their removal does not cause pain. Removal of the nasal packing is one of the most fearful things for patients. The fear is mostly fed by false facts. I had no patient, who felt pain while nasal packing is removed.
When can the patients return to their normal daily life after rhinoplasty?
It may take 10 days for the patients to return to daily lives after the surgery. The visible swellings and bruising on the patients’ faces disappear in this period. The patients have a nasal packing for the first 7 days after the surgery. After the packing is removed, the nose is covered with tapes. The aim of these tapes is to eliminate the swellings on the nose faster.
What are the considerations for the patients after rhinoplasty?
The patients should protect their noses for approximately 6 weeks after the surgery. The patients are recommended to avoid using eyeglasses, applying pressure on their noses while lying and to be careful to protect themselves from nasal trauma.
How long nasal spray should be used after rhinoplasty?
Patients are recommended to use nasal spray up to 3 months after nasal surgeries. Here, nasal sprays not only clean the nasal cavity, but they also help regression of intranasal swelling more quickly.
When can I see final shape of mu nose after rhinoplasty?
Shape of the nose may change throughout 1 year after rhinoplasty. The underlying cause is regression of intranasal swelling over time. Therefore, one year should elapse to see final shape of the nose after the surgery.
When can rhinoplasty be performed after nasal fracture?
One year should elapse after nasal fracture in order to undergo rhinoplasty. This period of time allows alleviation of trauma-related swelling in nose and makes the nasal injury visible in all aspects. The nasal damage will be completely obvious at the end of one year. The fibrotic tissues arising out of healing of the fracture will smoothen and the nose becomes operable again. For all these reasons, it is necessary to wait for 1 year after a nasal fracture.
Are secondary rhinoplasty procedures more challenging?
As is the case for all surgeries, rhinoplasty is much easier if it is performed for the first time. The procedures become more challenging comparing to the first surgery, if the patients are having their second or third rhinoplasty.
The most important reason for this is the adherences that occur in recovery period at the end of the first surgery. These adherences may hinder visualizing and exposing the bone and cartilage structures inside the nose.
Moreover, it is harder to reshape the cartilages in the secondary surgeries. Sometimes, cartilages removed from the nose are used to shape the nose in the first surgery. Therefore, sometimes, sufficient cartilage structure may not be found in the nose in the second surgery.
In these cases, the required cartilage is obtained either from the ear or the rib of the patient. This somewhat prolongs the surgery.
When can I start working out after rhinoplasty?
Patients should wait for approximately 2 to 3 weeks to start working out after the surgery. Here, the aim is to prevent swelling in nose secondary to blood pressure that increases during exercise.
